Any otherwise qualified student or prospective student who states that he or she has a disability as defined under the Americans with Disabilities Act of 1990 (ADA) or the guidelines for section 504 of the Rehabilitation Act of 1973 may register with this office.
To register for services, a student or prospective student must meet with the Counselor/ Special Needs or his assistant with appropriate documentation that supports a disability.
Eligibility Guidelines
The following guidelines are used in determining student eligibility for services provided through the Office of Disability Services. Students must follow these procedures to attain services. The Office of Disability Services reserves the right to deny services to any student who does not follow these procedures for obtaining services. Eligibility for other services not stated on this site will be determined on a case-by-case basis.
Documentation to Provide to the Counselor/ Special Needs:
• Doctor Statements (typed on letterhead)
• Multi-Factor Evaluations (within 3 years)
• Psychological Reports
• Letters of verification from the Bureau of Vocational Rehabilitation or other agencies
• Medical Records (family physician, clinic, etc.)
• Hospital Records
Consent to release information forms are available in Room 129, Office of Disability Services, to assist in obtaining documentation.
Appointments to Meet with the Counselor
Appointments are available Monday - Wednesday 9:00 AM - 3:30 PM, Room 129, Main Building, the Office of Disability Services. The student may be asked to provide the Counselor/Special Needs with documentation of his or her disability. This documentation is necessary for determining accommodations.
