• Currently admitted into a degree program and wish to add a certificate program.
• Currently admitted into a certificate program and wish to add a degree program.
• Currently admitted into a certificate program and wish to add an additional certificate program.

If you are admitted to a degree program and wish to add an additional degree, do not use this form.  Instead, complete the Double Major Form.
