- Teacher Checklist and Approval Request Form
High school faculty members who would like to be considered for approval to teach Cincinnati State courses for dual credit must complete this form and attach with it their resume and/or transcripts. This form and all attachments are returned to Off-Campus Programs for processing. A checklist is provided for your convenience.
- Dual Enrollment Application/Registration Form
This form is distributed to students by their high school teachers and must be completed in its entirety for admission into and registration for approved Dual Enrollment courses. Any student under the age of 18 must have parent or guardian signature.

Provides information regarding credit transfer, specific to the Ohio Board of Regents’ Ohio Articulation and Transfer Policy. Link provides resources pertaining to Transfer Assurance Guides (TAG) and Career-Technical Credit Transfer (CT)² courses, AP Placement, Ohio Transfer Module (OTM), and more.
