Cincinnati State Employee Vaccination Incentive Program

Cincinnati State is striving to provide a safe campus experience for everyone, by celebrating our employees who are vaccinated and encouraging all other College employees to get vaccinated against COVID-19.

Who can enter? What are the prizes?

Employees who are fully vaccinated, or have received their first dose of a 2-dose vaccine, can enter the weekly prize drawings.

Employees who are fully vaccinated are also eligible for the $5,000 Grand Prize drawing.

  • To enter, fill out the Employee Vaccination Incentive eForm (must be logged in to Blackboard/MyServices) with your name, and when and where you were vaccinated.
  • Attach to the eForm scanned copies of both sides of your vaccination card, or bring your vaccination card to Falonda Rodgers or Dina Henderson in the Human Resources Office on Clifton Campus (Main 186) for verification.

New incentives, effective Nov. 22, 2021:

  • Employees who complete the eForm between Nov. 22, 2021, and Dec. 17, 2021, and provide proof that you’re already fully vaccinated can receive a $50 gift card.
  • Employees who complete the eForm and provide proof that you’ve obtained (between Nov. 22 and Dec. 17, 2021) your first shot of a 2-dose vaccine, or obtained a 1-dose vaccine, can receive a $100 gift card.
  • Gift cards will be provided by the Human Resources Office at designated times.

When your vaccination info has been verified, you’re also eligible for weekly prizes that include:

  • Visa gift cards
  • Queen City gift cards for dinner/entertainment
  • Family dinner gift cards
  • Ticketmaster gift cards
  • 1-year fitness club memberships

Prize Drawing Dates

Each week from Nov. 1 to Dec. 20, eight fully-vaccinated or partially-vaccinated employees will win prizes in the weekly drawing.

On Dec. 20, one fully-vaccinated employee will win a grand prize of $5,000.

  • Weekly winners will be announced on Mondays about 11 a.m.
  • Winners can pick up their prizes on Tuesdays between 11 a.m. and noon in the Human Resources Office (Main 186), and then go to the Executive Offices (ATLC 343) to have their photo taken.
  • The grand prize winner will receive the $5,000 prize on Dec. 21.

Not yet vaccinated?

Get your COVID-19 vaccination from any authorized provider. Get information about locations throughout the Tri-State at https://healthcollab.org/vaccine-info.

  • You can fill out the eForm and enter weekly drawings after you receive your first dose of a two-dose COVID-19 vaccine.
  • After you get your second dose of a vaccine, you can take your completed vaccination card to Falonda Rodgers or Dina Henderson in the Human Resources Office on Clifton Campus (Main 186), and then you’ll be eligible for the grand prize drawing.

Cincinnati State Vaccination Incentive Program Official Rules

By entering the Employee Vaccination Incentive Program (“Employee Promotion”) offered by Cincinnati State Technical and Community College (“Cincinnati State” or “College”), you agree to be bound by these official rules.

Eligibility

This Employee Promotion is open to Cincinnati State employees working in a full-time or part-time assignment for the College during Fall Semester 2021. Those employees not fully vaccinated at the beginning of the Employee Promotion may enter at any time once they receive the first dose of a two-dose COVID-19 vaccine, until December 17, 2021. To be eligible for the grand prize drawing, the employee must be fully vaccinated.

Definitions

Fully Vaccinated

To be considered fully vaccinated, an employee must receive two doses of the Pfizer or Moderna vaccines, one dose of the Johnson and Johnson vaccine, or receive a World Health Organization approved vaccine.  

Employees

Cincinnati State employees working in a full-time or part-time assignment for the College during Fall Semester 2021.

Students registered for classes in Fall Semester 2021 who are also part-time employees of the College are not eligible to participate in the Employee Prize Drawing.

Instructors who teach Cincinnati State CCP classes at partner high schools but do not teach on a Cincinnati State campus are not eligible to participate in the Employee Prize Drawing.

Promotion Dates

The Employee Promotion begins October 26, 2021, and ends after the last contest drawing on December 20, 2021. Entries received after 5 p.m. EST on December 17, 2021, will not be considered for awards, regardless of whether the late receipt resulted from human, computer or transmission error. For individuals receiving the two-dose Pfizer or Moderna vaccine, both vaccine doses must be administered by the above 5 p.m. EST deadline to be eligible for the grand prize drawing.  

Entry Guidelines

Entry Process

The Employee Promotion is open to Cincinnati State working in a full-time or part-time assignment for the College during Fall Semester 2021.

To participate in the Employee Promotion, employees must complete the designated Promotion eForm and either attach copies of both sides of their vaccination card or bring the card to the Human Resources Office (Main 186, Clifton Campus). Vaccination status and employee status must be verified to receive a prize.

Individuals who have not been vaccinated can receive the vaccination on the Cincinnati State Clifton campus on designated Vaccination Clinic days. Individuals may also receive the vaccination from any authorized COVID-19 vaccination provider.

Prize Drawings

Winning names will be randomly selected by Cincinnati State staff each Monday from November 1, 2021, through December 20, 2021.

No employee may win more than one prize, except for the grand prize of $5,000, which may be won by a previous winner of a weekly gift card or other weekly prize.

Notification of Winners and verification of prize eligibility

Prize winners will be contacted by the Cincinnati State Human Resources Office after their eligibility has been verified.

The prize winner must provide their vaccination card to the Human Resources Office either by attaching to their Employee Vaccination Incentive eForm scanned images of both sides of their vaccination card, or by bringing the card to Falonda Rodgers or Dina Henderson in the Human Resources Office for verification.

The name and prize may be announced publicly on the Cincinnati State website, on the Cincinnati State Daily News website, or in other College communications.

If a winner does not respond to notification of the win within five (5) business days, the winner will be disqualified and a new winner will be chosen.

Conditions of Participation

Use of Submissions

By entering this Employee Promotion, the employee hereby grants to Cincinnati State the right to take their picture and to reproduce, use, exhibit, display, broadcast, distribute, modify, adapt, and create derivative works of photographs, videotaped images or video/audio recordings of the Contestant by incorporating them into publications, catalogues, brochures, books, magazines, photo exhibits, motion picture films, videos, electronic media, web sites, and/or other media, or commercial, informational, educational, advertising, or promotional materials or publications related thereto.

Cincinnati State reserves the right to modify or change the Employee Vaccination Incentive Program rules at any time. The Promotion is subject to all applicable United States federal, state, and local laws and regulations. 

Contest Ownership

Cincinnati State is the sole owner and operator of the Employee Vaccination Incentive Program. Official rules, terms and eligibility requirements, prize information, and selection are administered by the College.  

Waiver of Liability 

By hosting this Employee Promotion, Cincinnati State in no way endorses or sponsors any manufacturer of vaccinations. By entering this Employee Promotion, the employee is choosing to voluntarily receive the vaccination(s) and assumes all liability therefore. Cincinnati State is not responsible for any injuries or illnesses occurring as a result of an employee obtaining vaccinations and entering this Employee Promotion.

Additionally, any prize may be taxable and winners are responsible for payment of any taxes.
