I confirm that I am the person featured in this submission and that I am 18 or older. I give Cincinnati State Technical and Community College permission to use my name, photo, voice, likeness, video, quote, job title, and the story and information I’ve submitted, in whole or in part, to promote the college.

This includes TV, radio, web, social media, email, print, and paid advertising. Cincinnati State may edit for length and clarity. I will not be paid. I confirm I own or have permission to share any photos or videos I upload. This permission lasts until I withdraw it in writing at csmktg@cincinnatistate.edu. I understand that withdrawing stops future use but cannot undo materials already published or distributed.
